Official transcripts can be ordered via paper form. M.D. Transcripts forms can be downloaded from this site. Telephone requests are not accepted. Transcripts cannot be released if the student has a hold due to a financial obligation to the university.
Transcript requests must include the student’s signature, full name, and the name and complete address of the person(s) or organizations to which transcripts are to be sent. If final grades or degree statement are needed, a student may indicate that the transcript request is to be held until all requested data are posted. Requests require two (2) business days to process unless you have asked for a hold. Transcripts can be picked up in the Student Affairs office. Transcripts requested for pick up that are not claimed within 30 days of printing will be discarded and must be reordered.
Transcripts that require a form be enclosed (such as Licensure, FSMB, FCVS, etc.) must be requested with a PDF attachment copy of the necessary form included in the order. You must mail the MD Transcript Request Form (with additional forms attached) to the College of Medicine Registrar’s Office, 6850 Lake Nona Blvd., Orlando, FL 32827-7408, email to firstname.lastname@example.org, or drop off the request and forms in person at Suite 115 on the Health Sciences Campus at Lake Nona.
Please note that the UCF Registrar’s Office on the main campus cannot process transcripts for the M.D. Program. The UCF College of Medicine Registrar’s Office is unable to process UCF undergraduate or graduate transcript requests.
