Graduates of the University of Central Florida College of Medicine M.D. Program who require credentialing, board certification, and verification of their M.D. degree must submit the request in writing, including any forms, to the College of Medicine Registrar, 6850 Lake Nona Blvd., Orlando, FL 32827-7408 or comregistrar@ucf.edu

Please include the following information:

  • name (at time of enrollment)
  • complete return address
  • current phone number
  • complete address of any third party who should receive the requested documents

Transcripts: With the exception of charter class graduates and veterans, all alumni transcript requests must be submitted directly to Parchment. Charter class graduates and veterans should contact COMRegistrar@ucf.edu directly for a transcript fee waiver.

Diplomas and MSPE: To request a PDF copy of your diploma or a copy of your MSPE, please complete the Document Request Form and email to COMRegistrar@ucf.edu. Please note that only unofficial copies of the MSPE may be emailed to the student/graduate; official copies must be sent directly to the Board or the program(s) you are applying to.

ERAS applicants: If you are applying to a program in ERAS and need copies of your transcript and/or MSPE, a MIDUS request should be made listing COMRegistrar@ucf.edu as the contact. A link will then be sent directly to the Registrar for upload of documents (transcript, MSPE) directly into the ERAS portal.

Malpractice coverage: If the licensing agency or program requests proof of malpractice insurance and/or claims history while you were enrolled as a medical student, please complete the official request form.

If you did not graduate from the University of Central Florida College of Medicine M.D. Program, please go to http://registrar.ucf.edu/degree-verification. The University of Central Florida has contracted with National Student Clearinghouse to process all requests for verification of UCF undergraduate and graduate degrees, including request from UCF alumni.
