About Our Work
The Community Health Needs Assessment (CHNA) identifies the most significant health challenges facing our region and helps shape our Community Benefit strategy. We use CHNA findings to guide investments, support innovative solutions, and strengthen systems that improve access to care and health outcomes.
We work alongside community organizations to advance existing efforts rather than duplicate services. By building strong partnerships, we help direct resources to organizations that serve individuals and families facing the greatest barriers to health.
Community Health Priorities
Based on the most recent 2026 CHNA, our focus areas include:
- Access to Care
- Maternal Child Health
- Unintentional Injuries
How We Invest
We advance community health through strategic investments in local organizations and initiatives. Our funding supports programs that:
- Expand access to essential health services
- Strengthen referral pathways and care coordination
- Provide preventive screenings and health education
- Address social and structural barriers to care
Funding opportunities are offered through competitive grant process designed to support innovative, community-led solutions.
