Instructions

  • Complete CPD Application form
    • The CPD staff can offer assistance to ensure that the educational activity has been planned in compliance with ACCME Guidelines, Standards for Integrity and Independence, UCF Industry Relations Policy and Guidelines and CPD policies.
    • Incomplete applications will not be accepted and sent back to the course director to complete.
    • It is recommended that you submit your application materials at least 90 days before your activity date to ensure processing times.
  • Complete the CPD Disclosure form
    • This form is to ensure there are no relevant financial relationships that could cause a conflict of interest and influence the material presented. All persons involved with the creation of content, planners, speakers, moderators, and faculty will need to sign and complete this form.
    • Activity planners should submit this form with the application. All others can be collected after the application is approved by our committee.
      • If a relevant conflict of interest is noted, the CPD staff will reach out to mitigate it prior to the activity.
  • Submit the CPD application and all planner/faculty disclosure forms for processing.
    • E-mail completed materials to the CPD coordinator (Alaina West, alaina.west@ucf.edu)
    • The CPD Coordinator will review the application and forward it to the CPD advisory committee for approval and provide additional instructions once an approval decision has been made.

Please note: UCF CPD does not accept commercial support at this time. If any portion of your activity or event is funded by ACCME defined ineligible companies (pharmaceutical, etc.), we will be unable to be your CME provider.
