Please Note: For proper processing, all forms must be submitted to the Program Office in HPA II Room 312. 

Year 1

Use this form to obtain signatures from the three Faculty members, in whose labs you will rotate during the Fall and Spring semesters:

Once you join a lab, complete this Program of Study form with your mentor:
- PhD Biomedical Sciences Program of Study Form

These forms are for selecting your committee, or revising your committee composition once selected:
- Doctoral Committee-Candidacy Form   (required by Graduate Studies)
- Dissertation Committee Selection Form   (required by Program)

Use these forms for electives and research (e.g. dissertation hours, post-candidacy hours, etc.) registration as needed:
- Elective Registration Form
- PhD Grad Registration Form

Year 2

Bring this form with you to your dissertation presentation, which must occur by the end of the Fall semester in Year 2.  Your committee will use this form to vote on your written dissertation proposal, and by signing this form they implicitly vote on your oral presentation of said proposal:
- Dissertation Written Proposal Form

This form is needed for your annual meeting, which should occur in the Spring or Summer of your second year, and at the same time every year thereafter. At this meeting, you will update your committee on the progress of your dissertation research:
- Annual Review Reporting Form – Doctoral Progress to Degree Aug 2011

Make sure to bring these two forms with you to your candidacy exam, which must be completed prior to the summer of Year 2.  One is for the committee to vote on your written proposal, and the other is for voting on the oral presentation:

- Candidacy Exam – Oral Presentation Form

Year 3 and beyond

This form is needed for your annual meeting, which should occur no later than 12 months after your last meeting:

Use this form for research (e.g. dissertation hours, post-candidacy hours, etc.) registration as needed:
