State law (Florida Statute (1006.69) and Florida Administrative Code (6C-6.001-5)) requires all incoming students to provide written proof of adequate immunization to Measles (Rubeola) and German measles (Rubella). All M.D. students are also required to provide documentation of immunization to Mumps, Varicella, Hepatitis B, and Tetanus-diphtheria.
In addition, M.D. students are required to provide documentation of two-step PPD testing (documentation of two Mantoux skin tests within 12 months preceding enrollment at UCF COM, with the second test administered 1 – 3 weeks after the first test). If there is a history of positive PPD, documentation of a current chest x-ray and the tuberculosis screening questionnaire is required (within the past 12 months). Alternatively, Quantiferon-Gold TB test is also acceptable for those students with a history of positive PPD.
All enrolled students must show proof of a physical examination prior to matriculation into the College of Medicine. Please have your health care provider complete the following health physical examination documentation form and submit it to the UCF College of Medicine Office of Student Affairs.
