Transcripts

M.D. Transcript forms are available from the College of Medicine Registrar’s Office or can be downloaded from this site. Official transcripts cost for the M.D. program has been assessed as part of yearly fees. However, the transcripts cost for other UCF undergraduate or graduate programs is $10.00 per copy effective January 1, 2008.

To order official UCF transcripts:

• go to https://my.ucf.edu
• sign on using your PID and password. If you do not know your PID click on “What are my PID and NID?”
• once signed on, go to Student Self Service > Student Center > Academics – Undergraduate and Graduate Careers
• from the drop-down “other academic” click on “Transcript: Request Official” and follow the directions

Transcripts that require a form be enclosed (such as Licensure, FSMB, FCVS, etc) must be requested with a PDF attachment copy of the necessary form included in the order. You must mail the M.D. Transcript Form (with additional forms attached) to the College of Medicine Registrar’s Office, 6850 Lake Nona Blvd., Orlando, FL 32827-7408, or drop off the request and forms in person at Suite 115 on the Health Sciences Campus at Lake Nona.
